Disability Documentation Requirements
Documentation is required for all accommodations.
Documents provided must:
- Be on professional letterhead
- Denote absence/presence of disability
- Include the impact of the disability on daily activities
- Explain how the request may lessen or alleviate the impacts to daily activities
- Provider signature
